COVID Reporting Form – Clarification on when to complete

Out of love and courtesy to those who are coming to church in person, please use this form to report when someone has been diagnosed with COVID-19 and was present at church within 48 hours of symptom onset. Once the report is received, the church will contact you with additional information. Information provided here will be kept private. Thank you!
